DOH launches new online public portal for immunization records

HONOLULU (KHON2) — Need a copy of your official immunization records? Give this new online portal a shot.

The Hawaiʻi Department of Health Immunization Branch created an online portal that allows the public free, secure access to print and download immunization records.

As an individual, parent, or legal guardian, this online system can help you get proof of immunization for school, daycare, work, or travel.

This initiative is a part of the Hawaiʻi State Immunization System, which started in June 2025.

HiSIS is a cloud-based system that is updated daily by healthcare providers and stores immunization records of adults and children.

To access the HiSIS Public Portal, visit the website and submit a request. Once you do so, it will ask for a first and last name, birthday, gender and contact information.

When requesting records, contact information must align with the patient’s HiSIS profile.

DOH officials want to emphasize the importance of immunization information systems, as they help patients from all medical practices and healthcare providers stay up to date on vaccinations and gather immunization status data from the community.
